Hidroxicloroquina, ¿sí o no?

Desde hace una semana, hemos intentado abrir nuestro blog a otros temas y dejar de ser monotemáticos con la COVID-19, sin embargo los vergonzosos hechos ocurridos en los últimos días nos obliga a volver de nuevo al tema. En este caso queremos aclarar a nuestros lectores la utilidad, o quizás no, de la Hidroxicloroquina en el tratamiento de esta enfermedad.

Hydroxychloroquine, yes or not?

Since a week ago, we have tried to open our blog to other topics and stop being monothematic with the COVID-19, however the shameful events that occurred in recent days forces us to return again to the subject. In this case we want to clarify to our readers the usefulness, or perhaps not, of Hydroxychloroquine in the treatment of this disease.

Si nuestros lectores habituales repasan las últimas publicaciones de este blog, podrían comprobar que hace un par de meses hicimos un repaso a los tratamientos que se estaban proponiendo para intentar curar a los pacientes infectados por el coronavirus, en concreto en ese artículo hablábamos de la combinación de hidroxicloroquina con la azitromicina y la eficacia demostrada que esta combinación se estaba apreciando en algunas series de estudios, además alertábamos que el gran problema que se podría producir era el de las arritmias cardíacas que se podrían desarrollar, sobre todo en pacientes con alguna alteración cardíaca de base. Así mismo destacábamos el escaso número de participantes de dicho estudio, por lo que alentábamos a llevar a cabo estudios más amplios con estas sustancias.

If our regular readers review the latest posts on this blog, they may find that a couple of months ago we reviewed the treatments that were being proposed to try and cure patients infected with the coronavirus, Specifically in that article we talked about the combination of hydroxychloroquine with azithromycin and the proven effectiveness of this combination was being seen in some series of studies, also warned that the big problem that could occur was the cardiac arrhythmias that could develop, especially in patients with some basic heart disease. We also highlighted the small number of participants in this study, so we encouraged more extensive studies with these substances.

Pues en los siguientes meses, aparecieron nuevos estudios, con mayor número de población, nada menos que en dos prestigiosísimas revistas médicas como son The Lancet y The New England Journal of Medicine (NEJM). Por seguir una temporalidad de los hechos les diré que esto ocurría a finales de Mayo. En ella se establecían una poblaciones de estudio muy amplias. En concreto el estudio de la revista The Lancet hablaba de una población estudiada de más de 96000 personas, de las que casi 15000 pertenecían al grupo de tratados con hidroxicloroquina, sólo o combinado con un macrólido. Y sorprendentemente la publicación alertaba del mayor riesgo de muerte súbita de los pacientes tratados frente a los no tratados, asociado con un mayor riesgo de arritmia ventricular de-novo durante la hospitalización. Frente a esto, la revista NEJM publicaba que no servían de nada el uso de estos fármacos para prevenir o para curar una enfermedad con 4 o más días de desarrollo. Inmediatamente y a consecuencia de estos estudios, el director general de la OMS, Tedros Adhanom Ghebreyesusla, suspendía todos los ensayos clínicos que se estaban llevando a cabo en el mundo con esta sustancia (esto ocurría el 25 de Mayo) con el desconcierto mundial que esto provocó.

In the following months, new studies appeared, with a greater number of population, in no less than two prestigious medical journals such as The Lancet and The New England Journal of Medicine (NEJM). To follow a temporality of the facts I will tell you that this was happening at the end of May. It established a very large study population. Specifically, the study in The Lancet spoke of a study population of more than 96,000 people, of which almost 15,000 belonged to the group treated with hydroxychloroquine, alone or combined with a macrolide. And surprisingly, the publication warned of the increased risk of sudden death in treated versus untreated patients, associated with an increased risk of de-novo ventricular arrhythmia during hospitalization. In contrast, the journal NEJM published that the use of these drugs to prevent or cure an illness with 4 or more days of development was of no use. Immediately and as a result of these studies, the director general of the WHO, Tedros Adhanom Ghebreyesusla, suspended all clinical trials that were being carried out in the world with this substance (this happened on May 25th) with the worldwide confusion that this caused.

Curiosamente tanto los investigadores que publicaron su artículo en The Lancet como los del NEJM tomaron sus datos de unas bases de datos hospitalarias suministradas por una empresa multinacional, Surgisphere. A la vez que se publicaban estos dos documentos, la misma empresa promovía la utilidad de otra sustancia, en concreto un antiparasitario, la Invermectina. A raíz de estas publicaciones más de 100 científicos de todo el mundo, alertaron de que los datos que publicaban ambas revistas podrían estar falseados, ocultando la verdadera población a la que se habría estudiado, planteándose importantes inconsistencia en el tratamiento de dichos datos (por ejemplo, se habla en el estudio que en Australia se recogen 73 fallecidos cuando es mucho mayor de la cantidad registrada oficialmente). El estudio no da información detallada de los hospitales de cada país de los que provienen los datos, aseguran los firmantes. Utiliza además dosis de los fármacos que son de media 100 miligramos más altas de las que recomienda la agencia de medicamentos de EE UU. Frente a esto la empresa Surgisphere alegó que no podían dar sus datos ya que tienen contratos de confidencialidad con sus clientes. Algo muy raro estaba pasando. Todo esto ocurría el día 29 de Mayo.

Curiously, both the researchers who published their article in The Lancet and those from the NEJM took their data from hospital databases provided by a multinational company, Surgisphere. At the same time that these two papers were being published, the same company was promoting the usefulness of another substance, namely an antiparasitic, Invermectin. As a result of these publications, more than 100 scientists from all over the world warned that the data published in both journals could be falsified, hiding the real population that would have been studied, leading to significant inconsistencies in the treatment of such data (for example, the study states that 73 deaths are reported in Australia when this is much higher than the number officially recorded). The study does not give detailed information on the hospitals in each country from which the data come, say the signatories. It also uses doses of the drugs that are on average 100 milligrams higher than those recommended by the U.S. drug agency. In response to this the Surgisphere company claimed that they could not give out their data since they have confidentiality contracts with their clients. Something very strange was going on. All this was happening on May 29th.

Posteriormente a esto la revista The Lancet emite una corrección del artículo, modificando algunos datos de los registros publicados con anterioridad y como consecuencia de ello, la OMS anuncia el 3 de Junio la autorización para reiniciar de los estudios con hidroxicloroquina. Qué caos, ¿no les parece?. Por último y para rizar el rizo, tres de los cuatros autores del artículo de The Lancet obligaron a la revista a publicar la retractación pública de su artículo y por tanto a emitir una corrección de dicha publicación, en concreto el día 5 de Junio. En dicha nota, la revista informa que «Nuestros revisores independientes nos informaron de que Surgisphere no transferiría el conjunto de datos completo, los contratos de los clientes y el informe completo de auditoría de la ISO a sus servidores para su análisis, ya que dicha transferencia violaría los acuerdos con los clientes y los requisitos de confidencialidad. Como tal, nuestros revisores no pudieron realizar una revisión independiente y privada por pares y por lo tanto nos notificaron su retirada del proceso de revisión por pares.» De los autores, el único que no se retracta y no pide perdón por la publicación de estos datos «falseados» fue S.S. Desai, curiosamente fundador y director de Surgisphere Corporation.

After this, the journal The Lancet issues a correction of the article, modifying some data from previously published records and as a consequence, the WHO announces on June 3rd the authorization to restart the studies with hydroxychloroquine. What chaos, don’t you think?. Finally, to make things worse, three of the four authors of the Lancet article forced the magazine to publish a public retraction of their article and therefore to issue a correction of the publication, specifically on June 5. In that note, the journal reports that «Our independent reviewers informed us that Surgisphere would not transfer the complete data set, client contracts and full ISO audit report to their servers for analysis, as such a transfer would violate client agreements and confidentiality requirements. As such, our reviewers were unable to conduct an independent, private peer review and therefore notified us of their withdrawal from the peer review process.» Of the authors, the only one who did not recant and apologize for the publication of this «falsified» data was S.S. Desai, curiously enough the founder and director of Surgisphere Corporation.

Como siempre, en todas las guerras hay alguien que saca beneficios, en este caso la empresa Surgisphere, que trataba de implantar su propio tratamiento para la COVID-19, frente al extendido general de usar la Cloroquina o sus derivados. Para ello ha falsificados datos y curiosamente dos revistas de gran prestigio han caído en la tentación de publicar (quizás por hacerlo antes que nadie y apuntarse el tanto) estos datos de forma burda, perdiendo así todo su prestigio editorial. A partir de ahora, lo que desde amplia-mente.com tenemos claros son dos cosas, la primera que si bien no sabemos aún la eficacia de la cloroquina y sus derivados frente a la COVID-19, lo que si está claro es que su eficacia y su seguridad están más que demostradas, ya que a diario se usan millones de tratamiento con estas sustancias en el mundo (en tratamiento frente al paludismo o frente a enfermedades reumáticas) y por tanto no habría que dudar de su farmacoseguridad.

As always, in every war there is someone who makes a profit, in this case the company Surgisphere, which was trying to implement its own treatment for COVID-19, as opposed to the general spread of using Chloroquine or its derivatives. To do this, they have falsified data and, curiously, two prestigious magazines have fallen into the temptation of publishing (perhaps because they did it before anyone else and scored) this data in a crude way, thus losing all their editorial prestige. From now on, what we at amplia-mente.com are clear about are two things, the first is that although we still do not know the efficacy of chloroquine and its derivatives against COVID-19, what is clear is that its efficacy and safety have been more than demonstrated, since millions of treatments with these substances are used daily in the world (in treatment against malaria or rheumatic diseases) and therefore there should be no doubt about their pharmacosafety.

But in second place, what we have learned from this situation is that from now on we must go deeper into the critical reading of scientific articles, wherever they come from and the magazines that publish them have all the prestige in the world, we will have to look at them with a magnifying glass. We will continue to be attentive as always.

Pero en segundo lugar, lo que si hemos aprendido de esta situación es que a partir de ahora deberemos de profundizar en la lectura crítica de los artículos científicos, vengan de donde vengan y tengan las revistas que los publican todo el prestigio del mundo, habrá que mirarlos con lupa. Seguiremos atentos como siempre.
