Como sin duda sabrán a día de hoy, ayer día 29 de octubre se celebraba el día internacional del ictus. Hacemos esta afirmación en base a la excelente campaña publicitaria emprendida desde los medios de comunicación.

Con el término ictus, ya conocido a nivel popular como lo pueda ser el termino de infarto de miocardio, se denominan lo que en terminología médica son los accidentes cerebrovasculares (ACV), ya que su presentación es súbita e inesperada.
With the term stroke, already known at the grassroots level as it can be the end of myocardial infarction, are called what in medical terminology are the strokes (CVA) since its presentation is sudden and unexpected.
Dentro de los ACV, podemos a su vez encontrar dos tipos totalmente distintos. De una parte tenemos los ictus isquémicos (el equivalente al infarto agudo de miocardio) en el que se produce una obstrucción súbita de un vaso sanguíneo arterial de modo que se interrumpe el aporte de oxigeno a las células nerviosas que forman el cerebro y se produce una muerte de las mismas que a efectos prácticos es irrecuperable. El otro gran grupo de ictus son los accidentes cerebro vasculares hemorrágicos en los que se produce una rotura de un vaso sanguíneo que a su vez da lugar a una extravasación o salida de sangre que ocupa los aledaños a esa rotura y produce un destrozo de los tejidos neuronales que al estar contenidos en una cavidad inextensible como es el cráneo sufren una compresión de carácter irreversible. En muchas ocasiones, el ACV isquémico da lugar a una muerte tisular de la zona por falta de irrigación, de modo que daña a la propia arteria obstruida que se termina rompiendo y dando lugar a un ACV hemorrágico posterior.
Within the CVA, we can also find two totally different types. El ictus, en un porcentaje que supera la mitad de los casos da lugar a efectos muy indeseables, pues un tercio de los afectados fallece y otro tercio sufrirá secuelas de por vida. Precisamente iniciativas como la de este día internacional pretenden dar a conocer esta enfermedad para que hábitos de vida saludables supongan una prevención en su presentación y llegado el desgraciado caso en que irremediablemente se presente, se conozcan los síntomas precoces que hagan que el afectado busque ayuda profesional en la ventana de tiempo inicial en la que la lesión neuronal es todavía reversible o al menos asumible desde el punto de vista de las secuelas, consiguiendo así la recuperación total.

¿Cuáles son los síntomas que nos deben llevar a sospechar el inicio de un accidente cerebro vascular? Es muy importante su conocimiento, pues la asistencia médica precoz puede suponer la diferencia de que hablemos de un accidente isquémico transitorio o una muerte por ACV.

1.- Dolor de inicio brusco e inesperado. Muy intenso.
2.- Perdida brusca de visión, total o parcial que puede ser en uno o en ambos ojos.
3.- Alteraciones del habla. Cuesta hablar, ser entendido en definitiva.
4.- Pérdida de fuerza a algún nivel, como los anteriores de inicio brusco. Suele darse en cara y/o extremidades del mismo lado del cuerpo. Generalmente este es un signo muy visible para las personas que nos acompañan, pues la expresión facial se “dobla” significativamente a un lado.
5.- Aparejada a la pérdida de fuerza, también suele aparecer un “hormigueo” en determinadas zonas de la mitad del cuerpo.
6.- A veces, todo ello y dependiendo de la zona afectada, se pueden presentar sensaciones vertiginosas bruscas.

4.- Loss of strength at some level, like the previous ones of abrupt beginning. It tends to occur in face I tips on the same side of the body. This is usually a very visible sign for those who accompany us, as facial expression is «folded» significantly to one side.
5.- Rigged to the loss of strength, also often appear a «tingling» in certain areas of the half of the body.

Por supuesto, es indispensable que ante la presentación de este tipo de sintomatología, se solicite ayuda profesional urgente pues a mayor rapidez de actuación hay mas probabilidades de recuperación del cuadro, llegando a haber un porcentaje de pacientes que lo hacen sin secuelas. La ventana temporal de tres horas es crítica en la aplicación eficaz del tratamiento.

From the point of view of legal medicine, whose one of the workhorses is the diagnosis of the causes of death when by its rapidity and by its unpredictability makes it suspected of criminality, stroke is one of the pictures more frequent in the diagnosis of death in judicial autopsies.

Desde el punto de vista de la medicina legal, cuya uno de los caballos de batalla es el diagnóstico de las causas de la muerte cuando por su rapidez y por su imprevisibilidad la hace sospechosa de criminalidad, el ictus es uno de los cuadros mas frecuentes en el diagnostico de muerte en las autopsias judiciales.
Iniciativas como la desde día mundial del ictus, son una excelente iniciativa para poner coto a esta enfermedad.
