La cruda realidad golpea sin respetar tiempos, geografía o circunstancias. AMPLIANDO, suele traer los artículos mas importantes de la temporada a juicio de los lectores, a fin de que aquellos que en vacaciones aprovechan para leer temas de su interés, tengan una nueva oportunidad para hacerlo. Pero el derrumbe de un viaducto en Génova (Italia), nos hace rescatar una reflexión de 2014 sobre catástrofes, que sigue de plena vigencia a día de hoy. Como o suele decir, “se trabaja como se entrena”.
Simulacros, ¿improvisación o adaptación? / Simulations, improvisation or adaptation?
El entrenamiento en épocas de paz, bonanza, tranquilidad, es el mejor arma contra las contingencias cuando las circunstancias nos obligan a enfrentarnos a las desgracias. Esta es una realidad incuestionable.
Training in times of peace, calm, tranquility, is the best weapon against the contingencies when circumstances force us to confront the misfortunes. This is an unquestionable reality.
Desgraciadamente, a pesar de ser un concepto en el que toda la población está de acuerdo, está sujeto a múltiples interpretaciones. Y es esa disparidad de criterios y opiniones los que hacen que surjan los problemas.

Precisamente para evitar las contingencias negativas y minimizar hasta llegar a un número mínimo o nulo de ellas, se planifican los entrenamientos o simulacros en prácticamente todos los órdenes de la vida. Es curioso como en actividades a priori relacionadas con el ocio como pueda ser la práctica deportiva o determinadas actividades artísticas (profesionales al margen, por supuesto), nadie duda sobre la necesidad y bondad de entrenar una y otra vez, ensayando sobre lo ensayado hasta que la actividad de turno queda “troquelada” en nuestro inconsciente de modo que se generan mecanismos de actuación automáticos ante determinados estímulos.

Curiosamente, no ocurre así en otras actividades que de entrada tambien todos estamos de acuerdo en que deberían ser prioritarias respecto a otras. Hoy nos referiremos a actuaciones sujetas a entrenamiento en relación a la vida, integridad corporal y/o salud de la ciudadanía.

En el día de ayer, 15 de octubre de 2014, se realizó un simulacro de accidente aéreo con pasajeros. Intervinieron gran cantidad de efectivos de Emergencias Andalucía, Cuerpos y Fuerzas de Seguridad del Estado, Empresa Pública de Emergencias Sanitarias, Bomberos (de Málaga y del Consorcio Provincial), Consejería de Medio Ambiente de la Junta de Andalucía, Policía Local y Cruz Roja de Málaga y Alhaurín de la Torre, Colegio de Psicólogos, colectivos internos del Aeropuerto de Málaga e Instituto de Medicina Legal de Málaga. Este simulacro, entre otros asuntos a entrenar para los intervinientes, sirvió para entrenar la puesta en marcha del Protocolo nacional de actuación Médico – Forense y de Policía Científica en sucesos de víctimas múltiples.
On the day of yesterday, 15 October 2014, a simulation was carried out in air crash with passengers. Intervened large amount of effective emergency in Andalusia, State Security Forces, Public Health Emergency Service, fire fighters (from Málaga and the Provincial Consortium), Ministry of the Environment of the Junta de Andalucía, Local Police and Red Cross of Malaga and Alhaurin de la Torre, the College of Psychologists, internal collectives from Malaga Airport and the Institute of Legal Medicine in Malaga. This simulation, among other issues to train for the speakers, served to train the launching of the national protocol for action medical examiners and police scientific events in multiple victims.
Por supuesto, no hay nada que objetar a la realización de este tipo de ejercicios, y como es natural se detectaron (a título personal) durante la realización del ejercicio una serie de fallos o mejor dicho “puntos a mejorar”. Precisamente para esto están este tipo de ejercicios y creemos que éstos deben continuar hasta días después en los que mediante una puesta en común de los distintos intervinientes (todos, no solo coordinadores) para que se pulan aquellos puntos que a posteriori se piense que deberían haber resultado mejor.

La objeción surge cuando se puede observar como tras años realizando este tipo de ejercicios, quien hoy les escribe sigue observando como la escrupulosa aplicación del Real Decreto 32/2009, de 16 de enero, en el que se pormenoriza el anteriormente mencionado protocolo de actuación ante sucesos con víctimas múltiples. Con el texto del protocolo en la mano, cada una de los simulacros ha resultado ser un éxito por el que las instituciones implicadas vuelven un año mas a respirar tranquilas pensando que todo está cubierto ante la presentación de una contingencia de este tipo. A título individual, no estoy de acuerdo, y se lo explicaré a continuación.

Como todo protocolo emitido por un comité de expertos, se trata, se debe tratar de una guía de actuación genérica a aplicar en cualquier punto geográfico del territorio nacional, con múltiples características idiosincrásicas que los hace totalmente diferente entre si. Como miembro del Instituto de Medicina Legal de Málaga, les puedo asegurar que una vez en el lugar de los hechos sería capaz de reproducir el protocolo ante mortem (es el cometido encomendado en todos y cada uno de los simulacros en los que he intervenido) previsto en el Real Decreto, incluso en ausencia de los impresos contemplados en el protocolo establecido. Eso a estas alturas es un éxito de buena ejecución del protocolo, pero hay algo mas, siempre hay algo mas.

Para un médico forense, la participación en los equipos de identificación de víctimas comienza en el lugar de los hechos según el protocolo. Esto es un error. ¿Cómo debe llegar el/los médicos forenses al lugar de los hechos? ¿Hemos tenido en cuenta las mas que probables dificultades de movilidad que se pueden ocasionar en determinados puntos geográficos ante sucesos de esta magnitud?. Estos son solo dos elementos puntuales pero son una mínima parte de las dudas que se podrían exponer llegados al punto en que hubiese que aplicar el protocolo.
For a medical examiner, participation in victim identification teams starts at the site of the facts according to the protocol. This is an error. How should reach the/forensic doctors on the scene? Do we have taken into account the more than probable mobility difficulties that can result in certain geographical points before events of this magnitude? These are only two specific elements but they are a minimal part of the doubts could expose at the point that it had to implement the protocol.
Entendemos que una vez publicado en BOE el protocolo, deben ser las autoridades locales las que deben asumir su campo de conocimiento para adaptar la teoría general ideada, que no tiene por que ser igual en todos los lugares de la geografía nacional ni conocidos por todos los miembros actuantes llegado el momento. Una vez adaptado el protocolo, éste no debe quedar en la cabeza de aquellos que llegados el caso deben integrar el Puesto de Mando Avanzado, puesto que en el momento de aplicar todo lo ideado y ensayado, estos mandos no están para decir a cada uno lo que debe hacer sino para coordinar lo que cada uno de los actuantes ya deberían saber.

Esto que parece tan sencillo, tan simple y tan obvio, no lo es tanto. Imaginemos que tenemos a nuestro intrépido médico forense presto a actuar en un suceso de víctimas múltiples y se dan una serie de circunstancias adversas. ¿Y si ese médico forense acaba se acaba de incorporar al Instituto de Medicina Legal tras un concurso de traslados y no conoce la ciudad?, ¿cómo llega al punto de reunión si por ejemplo no tiene permiso de conducir (que les recuerdo no es obligatorio para aprobar las oposiciones de acceso al Cuerpo Nacional de Médico Forenses)?, ¿tiene o no tiene vehículo particular?, lo que es peor, ¿sabe exactamente donde debe ir?, ¿siempre es el IML el lugar donde debe estar el punto de reunión de los médicos forenses?, ¿Incluso si el lugar de los hechos es cercano al lugar de trabajo del medico forense o a su domicilio si el evento es fuera de horario laboral?. Así podríamos seguir hasta el infinito solo sobre el asunto de la llegada al punto donde deba ejercer su labor. Imaginen si consideramos el trasporte de material necesario, el reparto de funciones y todas las circunstancias que pudiesen surgir.

Todas estas consideraciones deben estar previamente contempladas y recogidas en un manual de actuación válido en el ámbito de actuación de los intervinientes. Si no es así, los miembros del Puesto de Mando Avanzado, no podrían estar de manera eficiente en lo que deben estar, que no es otra cosa que coordinar los equipos. Tal y como se plantea ahora estos mandos pueden estar mas tiempo ilustrando a sus equipos que coordinando y no debería ser así.
All these considerations must be previously referred to and reflected in a valid action manual in the field of action of responders. ¿El simulacro realizado ayer fue un éxito?, pues como casi todo en esta vida, depende desde el punto de vista que se mire. Creemos que una gran parte de su aplicación parte de la base de actuaciones que se improvisan sobre la marcha en relación al evento ocurrido y no debería ser así. Todos y cada uno de los miembros actuantes debe saber de antemano no solo aplicar su campo de actuación profesional (que por supuesto ya se sabe y se hace muy bien ), sino como llegar a hacerlo, en que circunstancias, donde y cuando hacerlo. Y todo esto, debería constar por escrito al igual que el protocolo general, adaptado a cada una de las provincias españolas, que admitámoslo, no constan de los mismos medios materiales y humanos cuando se comparan entre si, al igual que no tienen la misma cantidad de población que atender. Incluso si afinásemos mas, hasta debería constar documentalmente quien y como debe prestar servicios fuera de sus respectivos puestos geográficos de actuación en el caso de presentación de un determinado evento que pudiese exceder las capacidades de actuación local.
