Los acontecimientos recientes nos obligan a reflexionar sobre uno de los aspectos más difÃciles de nuestro trabajo diario. A raÃz del incendio de la torre Grendfell en Londres o más reciente aún, el incendio forestal de Pedrógão Grande en Portugal, se hace necesario llevar a cabo la identificación de múltiples vÃctimas carbonizadas (más de 70 cadáveres en el primero y por ahora, al menos 61 fallecidos en el de Portugal).
Identification of carbonized corpses: hard work.
Recent events force us to reflect on one of the most difficult aspects of our daily work. Following the fire of the Grendfell Tower in London or even more recently, the forest fire in Pedrógão Grande in Portugal, it is necessary to carry out the identification of multiple carbonized victims (more than 70 corpses in the first and for now, at least 61 died in Portugal).
En ambos casos, como decÃamos, se han producido gran cantidad de fallecimientos y la mayorÃa de ellos perecieron carbonizados. La acción del fuego sobre el cuerpo, va a provocar quemaduras de distinto grado, pero si se mantiene en el tiempo o actúan acelerantes, se puede llegar a la carbonización del cuerpo e incluso a la destrucción casi completa del mismo.
In both cases, as we said, there have been many deaths and most of them died charred. The action of fire on the body, will cause burns of varying degrees, but if it is maintained in time or act accelerators, it can lead to the carbonization of the body and even to the almost complete destruction of it.
La dificultad en la identificación indudablemente viene dada por la gran destrucción de los tejidos, generalmente nos encontramos un cadáver en la posición del boxeador (por las contracciones musculares y la retracción de los tejidos), la protusión lingual, desaparición del pelo o la coloración negruzca de la piel. Pero además a todo esto se le añada en muchos casos el estallido de las cavidades del cadáver, cráneo, tórax y abdomen, con la evisceración de su contenido. Por último, y dependiendo de la intensidad del fuego, las partes distales del cadáver van a ir desapareciendo, empezando por los dedos, de las manos o de los pies, a continuación las manos y el resto de los miembros, presentándose amputaciones postmortales.
The difficulty in identification is undoubtedly due to the great destruction of the tissues, usually we find a corpse in the position of the boxer (for muscle contractions and retraction of the tissues), lingual protrusion, disappearance of hair or blackish coloration of the skin. But in addition to all this is added in many cases the bursting of the cavities of the corpse, skull, thorax and abdomen, with evisceration of its contents. Finally, depending on the intensity of the fire, the distal parts of the corpse will disappear, beginning with the fingers, hands or feet, then the hands and the rest of the limbs, presenting postmortem amputations.
Las caracterÃsticas antropométricas y morfológicas del cadáver también han variado, es decir, el peso y la talla generalmente se reducen, es prácticamente imposible detectar cicatrices o tatuajes y los órganos sexuales hay que buscarlos en muchos casos en el interior del organismo. Además en los casos que hoy nos ocupa, multipliquen ustedes todas estas dificultades por 60 ó 70 individuos a identificar, la dificultad aumenta.
The anthropometric and morphological characteristics of the corpse have also varied, that is, the weight and size are generally reduced, it is practically impossible to detect scars or tattoos and the sexual organs have to be searched in many cases inside the body. In addition, in the present cases, you multiply all these difficulties by 60 or 70 individuals to identify, the difficulty increases.
Ante este panorama tenemos que establecer la identidad del individuo. Para ello debemos de basarnos en el estudio de aquellos elementos que pueden ser más resistente a la acción del calor, en este caso hablamos de los dientes y de los huesos. Para ello deberemos extraer la mandÃbula y llevar a cabo la realización del odontograma completo para intentar compararlo con la ficha dental de aquellos desaparecidos y que posiblemente sean los fallecidos. En estos casos podemos encontrarnos algunas piezas dentales destrozadas por el fuego, pero intentaremos confirmar la presencia de determinados tratamientos dentales para ver si coincide con los que tendrÃan en vida estos sujetos.
In this situation, we have to establish the identity of the individual. For this we must be based on the study of those elements that may be more resistant to the action of heat, in this case we talk about the teeth and bones. To do this we must extract the jaw and carry out the complete odontogram to try to compare it with the dental record of those missing and possibly the deceased. In these cases we can find some dental pieces destroyed by fire, but we will try to confirm the presence of certain dental treatments to see if it coincides with those that would have in life these subjects.
Otro elemento a estudiar es la realización y comparación de radiografÃas óseas de los cadáveres, en ellas trataremos de buscar deformaciones y/o alteraciones de los huesos, callos de fractura. O en el caso de una radiografÃa de los senos paranasales intentaremos compararlos con radiografÃas previas que nos pueda aportar la familia de los fallecidos. Asà mismo, a  veces el sujeto ha tenido en vida intervenciones de osteosÃntesis, por lo que en la autopsia de los carbonizados podemos encontrar placas metálicas o algunos de los elementos de osteosÃntesis utilizados y que llevarán un código que se puede consultar en la base de datos de la empresa fabricante y localizar el hospital en el que le ha sido realizada la intervención quirúrgica y asà encontrar la historia clÃnica y la identidad del fallecido.
Another element to study is the performance and comparison of bone x-rays of the cadavers, in them we will try to look for deformations and / or alterations of the bones, fractures callus. Or in the case of an x-ray of the sinuses we will try to compare them with previous radiographs that the family of the deceased can give us. Likewise, sometimes the subject has had osteosynthesis interventions in life, so that in the autopsy of the charred we can find metal plates or some of the elements of osteosynthesis used and that will carry a code that can be consulted in the database Of the manufacturing company and locate the hospital where the surgical procedure has been performed and thus find the medical history and identity of the deceased.
Por último y si todo lo anterior no ha tenido la suficiente fuerza identificativa, procederemos a la realización de los estudios genéticos. El material genético expuesto al calor extremo se va a desnaturalizar, dificultando su estudio. Es por ello que deberemos de obtener el material genético del fallecido de algunos de los elementos que resisten más la acción del calor, en este caso de la pulpa dental. Para ello remitiremos al laboratorio de biologÃa varios molares del fallecido del que se pueda extraer el ADN del mismo y compararlo con los familiares de los desaparecidos.
Finally, and if all of the above has not had sufficient identification strength, we will proceed to carry out the genetic studies. The genetic material exposed to extreme heat will become denatured, making it difficult to study. That is why we must obtain the genetic material of the deceased of some of the elements that resist the action of heat, in this case of the dental pulp. For this we will send to the biology laboratory several molars of the deceased from which the DNA can be extracted from it and compared with the relatives of the disappeared.
As we see, it is not an easy task that British and Portuguese forensics will have to face in the coming days. Meanwhile we can only say that we regret all these losses and that we stand in solidarity with the families of both tragedies.
Como vemos no es una tarea fácil la que tendrán los forenses británicos y portugueses por delante en los próximos dÃas. Mientras tanto solo nos queda decir que lamentamos todas estas pérdidas y que nos solidarizamos con las familias de ambas tragedias.
