La enfermedad de Alzheimer vuelve a protagonizar la actualidad editorial de amplia-mente.comamplia-mente.com. A pesar de lo que pueda parecer, según la población general cree que es la labor de medicos forenses en España (sede del blog), la enfermedad de Alzheimer llega a tener cierta importancia estadística en el trabajo realizado en los Institutos de Medicina Legal en base a las cada vez más frecuentes peticiones de exploración por parte de los Juzgados correspondientes, a fin de dirimir casos de internamientos involuntarios o procesos de incapacidad civil.

Alzheimer’s disease is once again the focus of the editorial news of amplia-mente.com. Despite what it may seem, according to the general public believes that it is the work of forensic doctors in Spain (headquarters of the blog), Alzheimer’s disease comes to have some statistical importance in the work done in the Institutes of Legal Medicine based on the increasingly frequent requests for exploration by the corresponding Courts, in order to settle cases of involuntary internment or civil incapacity processes.

Sus autores, han encontrado evidencias de que mediante una analítica de sangre, se podría detectar si se está acelerando el daño neuronal cerebral y por tanto se constituiría en una manera barata y sencilla de detectar si estamos ante un paciente con un importante riesgo de desarrollar enfermedad de Alzheimer.
Its authors have found evidence that by means of a blood analysis, it would be possible to detect if neuronal brain damage is accelerating and therefore it would be an inexpensive and simple way to detect if we are dealing with a patient with a significant risk of developing Alzheimer’s disease.
El marcador en cuestión sería la proteína ligera del neurofilamento (NFL), sustancia medible en sangre, a diferencia de otros métodos analíticos que implican la medición en líquido cefalorraquideo o la realización de resonancias magnéticas funcionales, métodos estos últimos, no muy asequibles en determinados entornos. Una simple extracción de sangre bastaría para obtener la proteína NFL, lo que ademas de facilitar las cosas, ahorraría costes.
The marker in question would be the neurofilament light protein (NFL), a substance measurable in blood, unlike other analytical methods that involve measurement in cerebrospinal fluid or the performance of functional magnetic resonances, the latter methods not very affordable in certain environments. A simple blood extraction would be enough to obtain the NFL protein, which in addition to making things easier, would save costs.
El fundamento de la prueba propuesta es que el daño neuronal, o llevado a su extremo, la muerte de la célula nerviosa, conlleva aparejado que la NFL se filtre al exterior celular, con lo que pasaría al líquido cefalorraquideo y posteriormente a la circulación sanguínea. Los niveles de NFL son acumulativos y los autores, encontraron que a mayor evolución de la enfermedad de Alzheimer, mas concentración de proteína se detectaba, lo que además fue usado como marcador de eficiencia de los tratamientos usados al efecto de ralentizar el avance de la enfermedad.
The basis of the proposed test is that the neuronal damage, or taken to its extreme, the death of the nerve cell, entails that the NFL filters to the outside cell, which would pass to the cerebrospinal fluid and then to the blood circulation. NFL levels are cumulative and the authors found that the greater the evolution of Alzheimer’s disease, the more protein concentration was detected, which was also used as a marker of efficiency of the treatments used to slow the progression of the disease.
Evidently, we are before a hopeful advance that will be applied in the assistance medicine, but without a doubt, the expert medicine, will benefit indirectly from such advances, because we are before objective findings that in the case of a hypothetical questioning in the oral trial, would have a lot of weight to arrive at the judicial truth of the case to settle, as they could be cases of civil incapacities in which the parts could be in disagreement on the magnitude of the degenerative picture of the presumed incapable one.

Evidentemente, estamos ante un esperanzador avance que será aplicado en la medicina asistencia, pero sin duda, la medicina pericial, se beneficiará indirectamente de tales avances, pues nos encontramos ante hallazgos objetivos que en el caso de un hipotético cuestionamiento en el juicio oral, tendrían muchísimo peso para llegar a la verdad judicial del caso a dirimir, com puedan ser casos de incapacidades civiles en los que las partes pudiesen estar en desacuerdo sobre la magnitud del cuadro degenerativo del presunto incapaz.
