El artículo anterior, Trastorno psicótico y cavum del septum pellucidum, versaba sobre la confirmación de algo ya conocido hace décadas en relación al estudio del sustrato morfológico de la psicosis. Hoy, les expondremos, estimados lectores, otra publicación que relaciona la psicosis con determinadas alteraciones estructurales del cerebro.
The previous article, Psychotic disorder and cavum of the septum pellucidum, was about the confirmation of something already known decades ago in relation to the study of the morphological substratum of psychosis. Today, we will expose, dear readers, another publication that relates psychosis with certain structural alterations of the brain.

From the first steps of the blog, the study of biomarkers, was of interest to their authors. In Biomarkers and psychiatry, we already exposed the great interest that a forensic psychiatrist can arouse, the existence of complementary tests in relation to a subject as subjective as psychiatry, whose clinical exploration is based almost exclusively on the subject’s report to inform and that in the judicial environment, so biased it may be, due to the great secondary gains that a diagnosis of mental illness can cause.
De muy reciente publicación según se escriben estas lineas, nos llega desde JAMA Psychiatry, el artículo Disorganized Gyrification Network Properties During the Transition to Psychosis, en el que los autores plantean la investigación sobre el uso de técnicas de imagen para detectar precozmente signos de psicosis en pacientes de alto riesgo, lo que supondría un tratamiento en fases muy iniciales, multiplicando exponencialmente el éxito del tratamiento instaurado.
From recent publication as these lines are written, comes from JAMA Psychiatry, the article Disorganized Gyrification Network Properties During the Transition to Psychosis, in which the authors propose research on the use of imaging techniques to detect early signs of psychosis in high-risk patients, which would mean treatment at very early stages, multiplying exponentially the success of the treatment established.
Desde el advenimiento de los grandes paradigmas psiquiátricos a partir de mediados del siglo XX, las corrientes de pensamiento dentro de la psiquiatría, se han alineado, fundamentalmente en dos grandes corrientes; las organomecanicistas, en la se postula la existencia de trastornos somáticos, cerebrales en su mayoría, y las organodinamistas, que ponen la atención, sobre todo, en la estructura psíquica como origen de la enfermedad mental, aunque reconociendo también que son los procesos orgánicos, los que ocasionan la desestructuración psíquica que lleva a la enfermedad mental.

Novedades como la expuesta en el artículo enlazado, no hacen sino confirmar aquellas hipótesis sin poder confirmar en aquellos tiempos iniciales de la psiquiatría contemporánea, dotando de pruebas objetivas ese sustrato orgánico que ya desde entonces se sospechaba.

La tecnología actual, es capaz de hacer visibles las conexiones entre regiones cerebrales y el descubrimiento publicado, revela que una alteración en las conexiones necesarias tras la girificación de la corteza cerebral que caracteriza al cerebro humano, y concretamente en el caso que nos ocupa, los autores, se centraron en observar como estas distintas regiones de la corteza cerebral se interconectaban entre si, de modo que concluyeron que era posible, en base a esta observación, predecir los pacientes de alto riesgo que podían desarrollar psicosis y los que no.

El estudio mediante resonancia magnética en tres grupos constituidos por sujetos sanos, sujetos ya psicóticos y otros sujetos sin psicosis, pero con riesgo de padecerla y que de hechos un porcentaje significativo, desarrolló posteriormente; demostró que en los sujetos psicóticos y en aquellos sujetos de riesgo que posteriormente al estudio desarrollaron psicosis también, existía una mayor disgregación de las redes neuronales que integran las interacción entre las distintas áreas cerebrales.
