Study of the origin of atrial fibrillation
We continue in the article that we present today, with cardiac pathology, after the presentation last Monday of the article Molecular bases in the pathogenesis of arrhythmogenic cardiomyopathy.
En muy raras ocasiones seguimos incidiendo en una temática casi idéntica en dos artículos seguidos, pero la actualidad editorial en el campo de la medicina así ha encajado la elección de la temática que les ofrecemos hoy.

En el caso que nos ocupa, la arritmia que origina el estudio que les presentamos es la fibrilación auricular, que si bien no suele provocar eventos de muerte súbita cardiaca por si misma, si que es una de las principales causas responsables de los accidentes cerebrovasculares por coagulación intracardiaca que dará lugar a coágulos sanguíneos que terminarán embolizando en vasos cerebrales y ocasionando la muerte del paciente en el peor de los casos.
In the present case, the arrhythmia that gives rise to the study we present is atrial fibrillation, which, although it does not usually cause sudden cardiac death events by itself, is one of the main causes responsible for stroke. Intracardiac coagulation that will lead to blood clots that end up embolizing in cerebral vessels and causing the death of the patient in the worst case.
La errática actividad eléctrica auricular que se da en la fibrilación auricular, se suele asociar en los países mas desarrollados a un envejecimiento cardiaco paralelo al propio envejecimiento de la sociedad. Surge por tanto la necesidad de estudiar las causas para ofrecer tratamiento para una patología cada vez mas prevalente.
The erratic electrical atrial activity that occurs in atrial fibrillation, is usually associated in a more developed countries to a cardiac aging parallel to the very aging of society. The need to study the causes to offer treatment for an increasingly prevalent pathology arises.
El principal escollo que nos encontramos para ello, es que los métodos habituales de estudio no presentan la eficacia deseada por falta de información suministrada en el caso del método no invasivo por excelencia, el electrocardiograma, o por las complicaciones clínicas que se presentan en el principal método invasivo que constituye el catererismo cardiaco.
The main obstacle that we encounter for this is that the usual methods of study do not present the desired efficiency due to lack of information provided in the case of the non-invasive method par excellence, the electrocardiogram, or the clinical complications presented in invasive methods.
En el estudio publicado en Plos Computational Biology titulado Novel non-invasive algorithm to identify the origins of re-entry and ectopic foci in the atria from 64-lead ECGs: A computational study, se plantea la realización de un algoritmo basado en señales de un chaleco que recoge 64 derivaciones frente a las 12 habituales para la localización del origen de la excitación auricular acelerada tras actividad focal ectópica, ondas de reentrada o ambas circunstancias a la vez. Se informa en el estudio la obtención de unas tasas de éxito de hasta el 93% en la localización del origen de la excitación anómala.
In the study published in Plos Computational Biology entitled Novel non-invasive algorithm to identify the origins of re-entry and ectopic foci in the atria from 64-lead ECGs: A computational study, the realization of an algorithm based on signals of a Vest that collects 64 leads versus the usual 12 for the location of the origin of the accelerated auricular excitation after ectopic focal activity, reentry waves or both circumstances at the same time. The study reports obtaining success rates of up to 93% in the location of the origin of the abnormal excitation.
Los autores concluyen que el algoritmo creado por ellos, supone un avance significativo para predecir los orígenes de la anomalía arrítmica en el caso de una única actividad focal auricular, pero aseguran que es posible identificar múltiples focos simultáneos usando los patrones de evolución del algoritmo, adecuados, justificando así investigaciones adicionales tras la senda iniciada en el estudio publicado.
The authors conclude that the algorithm created by them represents a significant advance to predict the origins of the arrhythmic abnormality in the case of a single atrial focal activity, but they assert that it is possible to identify multiple simultaneous foci using the appropriate algorithm evolution patterns, thus justifying additional investigations following the path initiated in the published study.
As we stated in the previous article, the investigation of causes usually leads inexhaustibly associated with the discovery of our treatment routes. Today we show a hopeful way of research aimed at the diagnosis of an arrhythmia very prevalent in our environment, which we hope will lead to a new advance in the development of specific treatments that ultimately lead to a decrease in morbidity and mortality associated with the pathology of heart rate.

Tal y como planteábamos en el artículo anterior, la investigación de las causas suele llevar indefectiblemente asociado el descubrimiento de nuestras vías de tratamiento. Hoy les mostramos una esperanzadora vía de investigación orientada al diagnóstico de una arritmia muy prevalente en nuestro medio, que esperamos lleve aparejada un nuevo avance en el desarrollo de tratamientos específicos que en última instancia den lugar a una disminución de la morbimortalidad asociada a la patología del ritmo cardiaco.
