No es la primera vez en la que en amplia-mente.com vemos la utilidad que últimamente se está dando a determinadas pruebas complementarias de imagen en la autopsia. Hoy añadimos un nuevo uso que podemos dar al TAC postmorten.
Using postmorten CT to determine the time of death
It is not the first time that we see in amplia-mente.com the utility that lately is giving certain additional imaging tests at autopsy. Today we add a new use that can give the postmortem CT.
Desde esta plataforma, hemos ido dando información acerca de los usos que se está haciendo de la TAC postmorten, por ejemplo para la identificación de cadáveres, para los ahorcados o en casos de sumersión, incluso hemos tratado de determinar si realmente es útil esta prueba diagnóstica y si se podrán evitar hacer las autopsias completas siendo sustituidas por las pruebas de imagen. En todos estos casos hemos llegado a la misma conclusión, que si bien las virtopsias (autopsias mediante técnicas de imagen) pueden ser útiles, no van a ser definitivas ni concluyentes, si no que al final deberemos de corroborar los datos recogidos en dichas pruebas mediante la apertura del cadáver. Sin embargo si van a complementar, orientar y ayudar en el diagnóstico final de la causa de la muerte o de la identificación del fallecido.
From this platform, we have been providing information about the uses being made of the postmortem CT, for example for the identification of bodies, for hanging or in cases of drowning, we have even tried to determine whether it is really useful this diagnostic test and if they can avoid doing full autopsies being replaced by imaging tests. In all these cases we have come to the same conclusion, that while virtopsias (autopsies by imaging) may be helpful, they are not going to be definitive or conclusive if in the end we will have to corroborate the data collected in these tests by the opening of the body. But if they are going to support, guide and assist in the final diagnosis of the cause of death or the identification of the deceased.
Hoy como decía añadimos a todos estas utilidades una nueva. Recientemente se ha publicado en el Journal of Forensic and Legal Medicine un artículo titulado «Estimation of post morten interval by tomographic images of intra-cardiac hypostasis» en el que se recoge la posibilidad de determinar la data de la muerte, mediante el estudio de las hipóstasis que se presentan en el interior del corazón. Este estudio se ha realizado en el Departamento de Patología de la Universidad de Sao Paulo (Brasil) y en él se han evaluado 23 cadáveres fallecidos en el hospital, a los que se le ha practicado una tomografía postmorten secuencialmente cada hora. En la ventana del mediastino, se ha evaluado la densidad de la pared anterior de la aurícula derecha e izquierda y se ha comparado con la densidad de la pared posterior de las mismas, estableciendo las diferencias de densidades entre ambas a lo largo del tiempo postmorten transcurrido.
Today as saying we add to all these a new utilitie. Recently published in the Journal of Forensic and Legal Medicine an article entitled «Estimation of post mortem interval by tomographic images of intra-cardiac hypostasis» in which the ability to determine the time of death is collected by studying the hypostases occurring within the heart. This study was conducted in the Department of Pathology, University of Sao Paulo (Brazil) and 23 deaths corpses have been evaluated in the hospital, obtained secuencially a CT at one hour of interval, postmortem sequentially. In mediastinal window it has been evaluated density of the anterior wall of the right and left atria and compared with the density of the rear wall thereof, establishing the differences in density between both along the elapsed time postmortem .
En base a un modelo exponencial de presentación de los datos, se ha podido establecer la data de la muerte con una diferencia menor de 2 horas, principalmente en las primeras doce horas tras el fallecimiento. Sin embargo, el presente estudio recoge que a partir de estas doce horas la relación entre la data real de la muerte y las diferencias de densidades de las paredes de ambas aurículas varía notablemente, siendo imposible establecer una relación proporcional, no siendo útil, por tanto, para la determinación de la data de la muerte.
Based on an exponential model of presentation of data, it was possible to establish the time of death with a minor difference of 2 hours, mainly in the first twelve hours after death. However, the present study reports that from these twelve hours, the ratio of the actual time of death and the differences in densities of both atria walls varies considerably, making it impossible to establish a proportional relationship, not being useful, therefore for determining the time of death.
As we have seen throughout all publications on our blog above, these additional tests can not avoid the traditional methods used for determining the time of death, as far the use of postmortem TAC will help in that diagnosis, in the first twelve hours postmortem, not being useful beyond this period. Again we corroborated that additional tests are helpful but can not replace traditional methods. We will continue to watch the evolution of these techniques.
Como ya hemos visto a lo largo de todas las publicaciones anteriormente mencionadas en nuestro blog, estas pruebas complementarias no pueden evitar los métodos tradicionales utilizados para la determinación de la data de la muerte, como mucho, la utilización del TAC postmorten va a ayudar en dicho diagnóstico, en las primeras doce horas postmorten, no siendo útil más allá de este periodo. Nuevamente corroboramos que las pruebas complementarias son una ayuda pero no pueden sustituir a los métodos tradicionales. Seguiremos atentos a la evolución de estas técnicas.
